FEES

Following are the fees for the most common eye examinations we provide:

Routine/Comprehensive Examination: $95.00

Contact Lens Assessment: $20.00
(of current contact lenses)

Contact Lens New Fit: $75.00-$125.00
(trying contacts for the first time)

Contact Lens Re-Fit: $30.00-$75.00
(switching from one type to another)

Filling out Forms: $20.00

These examination fees may be covered by Alberta Health Care or by other insurance plans, as follows.

INSURANCES

ALBERTA HEALTH CARE INSURANCE PLAN:

Children and Seniors:

Eye examinations for children and seniors are typically covered by Alberta Health Care. Some items NOT covered by Alberta Health Care for this age group include second opinions, contact lens assessments, filling out forms, dispensing fees or if the patient has already reached the maximum allowable visits by Alberta Health Care.

19-64 years of age:

Medically required eye examinations are typically covered by Alberta Health Care. However, routine eye examinations are NOT covered by Alberta Health Care for this age group. An eye examination may qualify as medically required due to a written referral from your family physician/“GP“, an emergency such as pain or sudden vision loss, or certain health conditions such as diabetes. If you require an exam because of an eye health concern and not simply because you are due for a routine assessment, Alberta Health Care typically covers the exam. These “medically required” exams generally do not include a prescription for glasses.

OTHER INSURANCE PLANS:

We direct bill the following insurance plans:

ASEBP

Blue Cross

QuikCard

Indian Affairs

Veterans Affairs

AISH

OVC

For all other insurances, you will be required to pay for your examination and then it is up to you to submit your receipt and any insurance forms to your insurance provider for re-imbursement. If your insurance form contains a section for your “service provider” to fill in, please bring the form to your examination.
